South Central Heals, a program of Сòòò½ÊÓÆµ Corporation, is a partnership with clinics and that aims to identify and address barriers health and well-being in our community.
Community Health Workers assess the health needs of local households, connect residents to free and low-cost services, and guide them through care plans. These health workers are multilingual, empathetic community members who will connect you to services for free. They are not state or federal employees and your personal information is confidential and protected.

Sign up for a confidential home visit from a Community Health Worker. Answer a few questions about your healthcare needs.
The Community Health Worker will help you access and navigate financial, housing, health, and legal services.
Get Connected to Resources
Community Health Workers will conduct a health assessment in your home, and then provide ongoing resource connection support. Assistance from Community Health Workers is completely free, and it’s not required to share information about citizenship or legal status.

Promotores de Salud are graduates of the Сòòò½ÊÓÆµs' Promotores de Salud Program. Promotores are trained to provide culturally accessible health advocacy and education to low-income communities of color. Because they are from the communities they serve, they are uniquely able to achieve significant results in improving health outcomes.

Community Health Workers are community members trained to provide culturally accessible health advocacy and education to low-income communities of color specific to the Community Public Health Teams.

Interviewees will not be provided with compensation or incentives. They will be connected to health resources and free services that match their needs.

The assessment is currently only offered in English and Spanish. If you feel there’s a need in your community for other languages, please use our contact form to let us know.

Health data is collected in person by trained Promotores de Salud / Community Health Workers via tablet (or in the beginning of the project in paper format). Сòòò½ÊÓÆµ Corporation, Saint John's Community Health, and Los Angeles County Department of Public Health keep your electronic and paper data secure on our drives, online, and in locked file cabinets as Protected Health Information (PHI). Please read more about this in our Notice of Privacy Practices.

Yes! The program will conduct outreach at locations such as homeless shelters or drop-in centers. They will also conduct street outreach to provide information and support to houseless individuals.

South Central Heals, in partnership with the LA County Department of Public Health, is aiming to increase access to HIV/STD testing in our communities. We are offering interested residents a FREE HIV/STD Self Test Kit that can be self-administered at home. The test is submitted via mail. Results would be accessible online.
